Book Voucher Request Form

By submitting this form, I understand:

· I must be enrolled for the term indicated above in the current academic year.

· I must be Pell eligible and must have turned in the required paperwork to Student Financial Services (SFS).

· I will receive a book voucher for no more than $400.00 of Pell grant award for any term.

· If my Pell grant credit balance is below $400.00, I will receive the available balance.

· Loan funds are not eligible for book voucher advances.

· If I am approved for a book voucher, I authorizes Frank Phillips College to advance a partial Pell grant disbursement prior to the first day of the class term. The amount will be posted to the student’s portal billing ledger and financial aid ledger.

· I am responsible for full repayment to Frank Phillips College of the amount advanced as a book voucher if the student does not attend the term indicated above in the current academic year.

· Book vouchers are only accepted through the first week of classes for each term.
